Student Learning Profile Program

For some students, additional support is needed within the classroom in order for their God-given gifts to shine. Accommodations and modifications are provided through our Student Learning Profile (SLP) Program.
Accommodation/Modification Policy
To receive accommodations or modifications at
A. A Psycho-Educational assessment performed by a psychologist
Or
B. A diagnosis by a professional in the medical field.
Medical professionals include:
1. Medical doctors 4. Physical therapist
2. Speech/Language Pathologists 5. Occupational therapist
3. Audiologists 6. Other:_______________
Assessment
The assessment/diagnosis of the child is the responsibility of the parent. In addition, it is the parent’s responsibility to provide MCS with a copy of the assessment/diagnosis report. This copy of the assessment/diagnosis report will be kept on file with the SLP Coordinator. It does not become a part of the student’s permanent record and is not forwarded to other institutions in the event the student transfers or graduates.
Once a current assessment report is provided to the school, a SLP Team will review the results and determine the accommodations and/or modifications that can be provided by MCS. The assessment is valid for 3 years. After that time, the parent must have the child reassessed in order to continue receiving accommodations/modifications at MCS.
SLP Team
A SLP Team will meet to develop a Student Learning Profile (SLP) based on the student’s individual learning needs. This profile will state the agreed upon accommodations/modifications the school will implement for the student. The SLP Team will meet after a parent has provided the school with a student’s assessment/diagnosis report and then annually to review and possibly revise the SLP.
Each team member is to receive a copy of the signed SLP. Teacher’s copies must be kept in their Confidential Student Record book for easy access and will be destroyed at the end of each school year. The SLP Coordinator will keep a copy on file as long as the child remains a student at MCS.
The SLP Team is made up of the following members:
SLP Coordinator Test Examiner (Optional)
Parent(s) Tutor (Optional)
Classroom Teachers Therapist (Optional)
Administrator Student, if appropriate
Report Cards
Documentation on student report cards will read: *Grades reflect modifications and accommodations as specified on SLP
Modifications – A change of assignment or test format (on grade level)
Examples include: reduce written assignments, allow the use of computer, break
into smaller units, abbreviated assignments, etc.
Accommodations – altering the method of delivery of teaching or testing (on grade level)
Examples include: extended time, separate setting, allow for scribe, provide a
copy of notes, give preferential seating, provide visual or verbal cues, etc.
The SLP Program is provided at a cost of $600 per year or $50 per month. Teaching Assistance by a Para-professional is also available at a cost of $10 per hour.
